# 06.01 Ubah Profil

Untuk mengubah data profil dokter, melengkapi alamat dan juga mengecek ID Satu Sehat Dokter.

 

### Cara Penggunaan :

 

#### Ubah Profile

1. Pada form ubah profil, Anda bisa mengubah data dokter, termasuk dokumen-dokumen

   - Nama Lengkap : Inputkan nama lengkap anda beserta gelar
   - Spesialisasi : inputkan spesialisasi anda
   - No. KTP : inputkan nomor KTP anda
   - Id Satu Sehat : klik tombol **Cari**, setelah anda menginputkan No. KTP anda
   - Scan KTP : upload file Scan KTP anda
2. Setelah itu anda scroll kebawah untuk melengkapi data selanjutnya

   - No. STR : inputkan nomor Surat Tanda Registrasi dokter anda
   - Scan STR : upload File Scan STR anda
3. Setelah anda scroll lagi kebawah

   - No. SIP : inputkan No. SIP anda
   - Scan SIP : upload file scan SIP anda
   - No. Whatsapp : inputkan nomor whatsapp anda
   - Alamat : inputkan alamat lengkap tempat praktik dokter anda
4. Setelah itu lengkapi detail alamat anda seperti provinsi, kota, kelurahan

   - Provinsi : pilih provinsi tempat praktik dokter anda
   - Kota : pilih kota praktik dokter anda
   - Kecamatan : pilih kecamatan tempat praktik dokter anda
   - Kelurahan : pilih kelurahan tempat praktik dokter anda
5. Selanjutnya

   - RT : inputkan RT tempat praktik dokter anda
   - RW : inputkan RW tempat praktik dokter anda
   - Kode Pos : Inputkan Kode Pos tempat praktik dokter anda
   - Pilih Titik Alamat :
      Pertama anda klik gambar mapnya

#### Ubah Akun

3. Anda bisa langsung mengubah email dan username dokter
4. Namun untuk mengubah password, centang ubah password terlebih dahulu

5. Lalu inputkan password baru, Konfirmasi password baru dan password yang lama
